Referral Manager

The Referral Manager is responsible for growing KenCrests' Lifesharing program through outreach and networking with internal and external stakeholders, marketing and overall promotion of KenCrest Services and Lifesharing.  This position encompasses all the duties involved in managing incoming individual referrals and recruiting potential Lifesharing families.

Job Responsibilities include but not limited to

  • Point of contact for all incoming referrals both internal and external with a response time of 48 hours to all inquiries.  Handle emergency referrals, placement and billing. Responsible for all steps of intake starting with referral, assessment, matching all the way through to placement.
  • Continuously build pool of available and vetted host family providers and respite providers.  Review completed and approved applications, schedule and complete home studies, coordinate visits, work to finalize match and placement.  Conduct introduction to Lifesharing training.
  • Serve as a temporary Lifesharing Coordinator for new placements coming into our program as needed.  Complete all required documentation to meet licensing and KenCrest Lifesharing standards/practices.  Provide necessary supports to those new individuals and families, to include attending monthly home monitoring and team meetings until another coordinator is assigned.
  • Develop and expand pool of host family providers that are able to provide temporary care internally and externally to our program.  Perform walk-through home inspections and interviews with potential host family providers only interested in providing temporary respite.
  • Complete pre-service trainings with new Lifesharing providers and respite providers and conduct Lifesharing presentations for potential host family providers interested in our program.
  • Update and maintain host family provider information in database and provide regular updates to management team
  • Promote Lifesharing program at various events and maintain the marketing materials and social media platforms in conjunction with Marketing Manager
  • Develop and complete various capacity-related reports
  • Responsible for all aspects of billing into the Evolv system.  Obtain all documentation from vendors, payment to families and referral bonuses to current host families. Develop new processes ralated to billing, processes and services for the program.


  • Bachelor's degree in related human services field
  • Marketing experience preferred
  • Residential Referral/Intake and Lifesharing experience, a plus
  • Two (2) years of related work experience with families and individuals with IDD
  • Clear criminal record and child abuse check in accordance with state requirements
  • Valid Driver's license.
