Nurse Coordinator - Community Medical Homes-$5000 Sign On Bonus

The Nurse Coordinator is responsible for assessing and supervising the nurse care managers who provide care management to the residents in the community living homes. This position will provide quality oversight of nursing practices and documentation. 

Essential Functions:

  • Provide direct supervision of nurse care managers
  • Ensure that all nurses adhere to the established Protocols, Procedures and Standards of Practice
  • Incorporate knowledge of care of individuals with Intellectual Disabilities and the Aging as well as knowledge of disease processes to assess and analyze delivery of nursing care
  • Mentor nurse care managers to ensure that the highest quality of care is provided to the residents
  • Oversee the day to day schedules of the nurse care managers
  • Provide ongoing critical review of nursing documentation
  • Ensure that medical information in Evolv is accurate
  • Oversee training provided to direct care staff by the nurse care managers
  • Recruitment and hiring of nurse care managers
  • Coordinate and conduct quarterly staff meetings
  • Maintain regular contact with the Director of Nursing via 1:1 meetings and scheduled management meetings to assure ongoing quality of service
  • Provide annual reviews of job performance for nurse care managers 
  • Develop disciplinary action as needed
  • Maintain familiarity with the state licensing and other compliance requirements
  • Develop and maintain health care training for agency staff
  • Serve as liaison between all healthcare team members, to ensure accurate and timely exchange of medical information between healthcare team and ancillary services.
  • Participate in agency committees and workgroups
  • Participation in administrator on-call rotation


  • Valid PA RN nursing license
  • Minimum of 5 years pediatric vent/trach experience
  • Maintenance of required 30 CEU hours/2 years
  • Current Healthcare provider CPR Certification
  • Valid Driver's License
  • Must have own mode of transportation
